家母的頭暈越來越厲害, 懇請指導！

家母81歲, 沒有長期的內科疾病或服藥. 偶發性的頭昏通常給予 cephadol, sibelium 等藥物一兩天就解決.
近兩週來頭昏的症狀復發, 程度與時間都較之前厲害, 甚至影響日常行動. 給予同樣的藥物給無明顯改善
懇請指導, 拜謝！

做了CT才知道有小腦萎縮與small multiple infarcts

今天也一個病人跑來說前天吃了我的藥後反而頭更暈。
她長期有梅尼爾症，耳鳴，高血壓，偶爾久久不舒服時才跑來打針、吃藥。
我這次開的藥有加感冒藥，病人很敏感，覺得吃了反而暈 (眼汪汪) ，只好跟她解釋是血壓高造成的，請她多來診所量血壓，打針… 。
後來才知道，她因頭暈怕到，藥只吃一包，昨天又給人家請，吃辦桌菜大餐，所以……

是自己人的話 MRI 排做下去 .. 若沒事 擬安心 若有問題 及早處理 ..別管啥規定先做啥 在做啥檢查 .. 直接 definitive Dx (水草舞)

自己人就不用想太多
不管dizziness, vertigo.....
首先就是排除central lesion
做就對了
negative再來用藥物治療

1.24hr Holter ECG.
2.cardiac echo.
3.neck CT angio(64 slice CT).

家母的症狀對sibelium cephadol 一樣沒反應
越檢查越嚴重
越問越不舒服
歸於psychogenic

Neuro 中有診斷
Vertebrobasilar insufficiency
雖然書上會說要有 Focal sign
但不一定
MRI + MR angio or CT angio 先做
有 finding -- OK
沒 finding 也不能排除 VBI
PS. subclavian atery stenosis 也要考慮

0. 對不起-補充-老教授以前指導-先排除metabolic imbalance induce-抽血-驗一下電解質以及血糖等等..
1. ECG- 排除arrhythmia
2. EEG- 先找可能異常放電病灶
3. MRI- DDx lacunar infarction
4. Dopascan (neck +transcranial) DDx carotid a. stenosis or

昨天多給一顆 Euclidan, 有明顯改善.

小弟趕快來回....
有時候subcortical lacunar infarct不見得會有focal sign....
(但是有75%的pt BP會高起來....)
pt可能持續Dizziness....MRI might be helpful....
遺憾的是....Anti-platelet, anti-coagulation, Fluzine, BZD, anti-histamin可能都無法改善dizziness....
Neuro的觀點 Dizziness是cerebral flow failure....
Including:
1. Cerebral vascular arthrosclerosis ( ECCD/MRA can be considered )
2. Hypotension or Posture Hypotension
3. Anemia
4. Heart failure or Cardiac arrhythmia
但是門診常看到的卻是Non-organic的問題....
我覺得這可能是Normal 到 Tension headache中間的症狀....
常常是因為睡眠狀況造成的....
I prefer Muscle relaxant/beta-blocker (response in short termed) or Flunarizine (response after long termed usage) in this situation.
Heat packing might be helpful....

老媽兩年前開始偶爾 仰頭會diziness ,fainting(原本就有vertigo)
一直以為是自律神經失調
看了好幾個ent未果
後來還是排了brain MRI
----->meningioma

我記得神外很厲害
做angiography進去把Plaque 拉掉，或是跟心導管一樣放個Stent
或是放有濾網的Stent
術後一樣吃anti platelet
不過現在如何

先從CNS查起, 若MRI是negative就放心多了

一位82歲男性,沒特別systemic disease,因睡眠障礙(不易入睡,易醒)來求診,自述每天早上醒來一定會頭暈一小時,靜坐在藤椅上,慢慢好了,其餘時間活動正常,已十年左右,建仁H及住處附近診所治療均未改善,如何進一步處理?

1:有S/s加上狹窄超過50% 或
2雖沒症狀 但挾窄超過70%
可置放stent(支架)啦
