從美國歐巴馬健保改革受阻,將面臨自動減赤風暴,;看台灣政局!!
美國政府是在這月一日,正式被國會(參 眾院)退回2014年預算案(因包裹歐巴馬的健保改革法案中的保險交換-Insurance Exchange),這是歐巴馬健保改革方案中,可預期的難關,所已列入在最後(2014年)完成,代表著民主黨與傳統共和黨的基本價值觀與信仰是不同的,共和黨是代表華爾街,擁有悠久政治,經濟勢力的保守勢力,所以要看歐巴馬健保改革方案,勢必從歐巴馬當選時,2008年藉參院通過美國"振興經濟與再投資法案"說起,;....
在振興經濟與在投資法案中,有相當大的預算是執行傳統醫療指標(效益比-cost-effective 改為Quality Of Life-QOL;;簡單說就是醫療不單是救活病人,而是以恢復有意義的生活-回歸社會, 家庭為目的);.........
以後才有Affortable Care Act-ACA 可負擔的照護法案在參院通過(2011年,在驚險中通過---民主黨參議員與共和黨參議員連署通過將墮胎列入ACA法案中,使得參院創紀錄的開會 討論到早上4:30才通過;......以下有期中選舉的民主黨大敗,歐巴馬在驚險中連任成功(期中有債務上限與財政懸崖問題阻擾都過關了),但是自動減赤(目前面臨考驗);. 歐巴馬連任成功已縮減聯邦預算1.3兆(?),裁減聯邦公務員18萬人了!!!...........
當然健保改革法案中的保險交換-Insurance Exchange是牽涉保險公司, 醫療提共者--provider,保險人- insurer參者之間利益從分配(?),;這是資本主義自由經濟市場醫療照護的公平 正義性,也是維護基本人權的範疇,所以要能被各方接受,不是英國 歐洲的社會主義的公醫制度所無法取代的(?)……

On October 1, the health insurance exchanges created by the Affordable Care Act (ACA) will open for business. Administrators must complete myriad tasks if they are to serve the millions of individuals — many of whom will be eligible for subsidies — and employees of small businesses who are ready to buy insurance……
First, the scope of exchange coverage will grow. Starting in 2016, exchanges will have to offer insurance to businesses with 51 to 100 employees. They have that option now, but no state-run exchange will exercise it in 2014
Second, states now have the authority to createTh a single, unified market for insurance by barring insurance sales to individuals and small businesses outside the exchangeThird, the quantity and quality of information on the prices charged by different hospitals, physicians, and other providers and the availability of data on the quality of care are improving
Fourth, the ACA has set in motion a large number of pilot programs, experiments, and demonstration projects involving new methods of paying for care and organizing providersFifth, few states have given exchanges the power to contract selectively with particular insurers or health delivery organizations, and fewer exchanges have fully used the power they have. Only six states are requiring insurers to offer standardized plans, and some of the minority of states that are limiting the number of plan offerings have set the cap so high that consumers are likely to experience information overload

Conclusion: The published literature lacks strong evidence that organic foods are significantly more nutritious than conventional foods. Consumption of organic foods may reduce exposure to pesticide residues and antibiotic-resistant bacteria.

…But Barack Obama dismissed such criticisms earlier this week in a Rose Garden speech he made on Monday to mark the launch of the exchanges.
….. Under the Affordable Care Act people who do not obtain qualified health coverage in some form in 2014 will be penalized 1% of their yearly income or $95 per person in 2014, whichever is higher. The penalty increases every year until 2016, when it will be 2.5% of income or $695 per person, whichever is higher.
A recent Gallup poll showed that 65% of those surveyed who currently lacked insurance said they intended to purchase insurance rather than pay the penalty, but a majority said that they were “not familiar at all” with the exchanges………

其實不需要下周二(1-2-10-15)民進黨提"倒閣案",等到10-17 歐巴馬與眾院協商後結果,就會對國民黨立委形成巨大壓力, 到時候會不會跑票,就立見真章!!!

Bumler連任(2012)就拋出油電雙漲,但是台電與中油弊端,卻不聞不問?!造成通貨膨脹(政府的基本經濟責任!!!).....勞保虧空就轉移話題為軍, 公, 教與18%問題(最近十年退休與將休的,已改了許多,--例如18趴就取消了!!!)......但是最不公平的學官雙棲與黨職換公職(?!)有改嗎?!.....這就是社會公平 正義的事;--無怪乎1985公民要發動數次遊行 抗議活動!!!

The federal government has long used taxes to achieve public health goals, but in fairly limited ways. Taxes and tax penalties for individuals have generally been confined to products that cause health harms and associated social costs, such as tobacco, alcohol, firearms, and pollutants……假借"防癌”,增加菸類, 酒類, 空汙等的健康捐(就是稅!!!),所以呢?!........

台灣還在馬王政爭,對於經濟一點都不關心, Bumbler2008年任台灣總統,就油電雙漲,造成通貨膨脹!!!.....2012年連任成功,電價又驚夏季電漲,今年十月又漲,似乎通貨膨脹與政府一點關係 都沒有!!!......美國談健保(?!)支出,首先談Inflation,因為通貨膨脹是百業都有關,!!!.........通貨膨脹就影響經濟活動(消費減少, 政府還是稅收減少!!!....社會投資就減少的惡性循環!!!)......

Andrew Percy, Conservative member of parliament and member of the health select committee, was reported by the Daily Mail as saying, “Doctors do work very hard, and are very dedicated to their training but to say they should be paid more is a bit cheeky.In the Daily Telegraph Matthew Sinclair, chief executive of the Taxpayers’ Alliance, said of general practitioners’ salaries: “Doctors’ focus should be on treating patients, not cashing in at taxpayers’ expense.”
Doctors train for years to provide an important service. We need to be up front that we are expensive because we are a skilled profession. Our message to the public should be this: you don’t want a doctor on the cheap.
